BACKGROUND: The angle of shoulder slope has been reported in accordance with the specific occupational activities of the aviation and textile industries. However, as no accurate definition nor standardised anatomical landmarks exist within the medical field, this study aimed to devise an appropriate definition with preplaced reference landmarks. In addition, the vertebral level of the acromial tip was also determined. METHODS: The sample series comprised 260 posterior radiographs of the shoulder, of which 127 were males and 133 females. The ethnic distribution included ten black, 13 coloured, 49 Indian and 188 white individuals. In accordance with the trapezial line, the angle of shoulder slope was defined and measured as the angle between the line from the spinous process of C7 to the acromial tip and the line from the acromial tip directly across to the median plane of the vertebral column. RESULTS: The standard mean angle of shoulder slope was approximately 13.56±3.70°. Left and right sides appeared to have mean angles of 13.81±3.41° and 13.33±3.95°, respectively. Mean angular values were also calculated in accordance with the demographic representation - sex: male 13.64±3.71°, female 13.48±3.71°; ethnic groups: black: 13.81±3.81°, coloured: 12.18±3.82°, Indian: 12.97±3.09°, white: 13.64±3.96°. Although the acromial tip was commonly aligned to the level of the spinous process of T3, the incidence of the vertebral level of the acromion was categorised into seven groups, viz. i) intervertebral disc between T1 and T2; ii) intervertebral disc between T2 and T3; iii) intervertebral disc between T3 and T4; iv) spinous process of T1; v) spinous process of T2; vi) spinous process of T3; vii) spinous process of T4. CONCLUSION: Since a statistically significant P value was recorded for the comparison between the angle of shoulder slope and the acromial vertebral level, it was postulated that the magnitude of the angle may determine the acromial vertebral level. As the present study incorporated standard osteological landmarks into the definition and calculation of the angle of shoulder slope, it may provide reference data regarding the position of the acromion which may be indicative of shoulder asymmetry and distorted shoulder setting. BACKGROUND. HIV/AIDS-related Kaposi's sarcoma (HIV-KS) is a public health problem in South Africa (SA). It is AIDS defining. There have been no studies evaluating its prevalence since the national roll-out of highly active antiretroviral therapy (HAART). OBJECTIVE. To evaluate the effect of HAART on the disease profile of HIV-KS in KwaZulu-Natal Province (KZN), SA. METHODS. Charts of patients with histologically confirmed HIV-KS were reviewed at an oncology clinic in KZN. The significance of associations of HAART with age, gender, CD4 count, urban/rural residence, fungating lesions, ulceration and lymphoedema, and treatment delay, was determined by t-tests for normally distributed continuous variables and χ² tests for categorical variables. Logistic regression models were used to analyse the association of HAART with CD4 count. RESULTS. Of 198 patients, 194 were documented as HIV-positive; 168 (86.6%) were on HAART at the time of their KS diagnosis. The mean CD4 count of 266 cells/μL was higher than that in previous studies at this site. The mean age at presentation was 36.6 (standard deviation 10.1) years. Females presented at a younger mean age than males (p<0.001). The mean age of females on HAART was 34.7 years and that of males 39.0 years (p=0.003). HAART-naive patients were three times more likely than those receiving HAART (15.4% v. 4.8%) to have visceral involvement (p=0.03). CONCLUSIONS. HAART use has resulted in outcome improvement. Mean age at presentation has increased in the group as a whole and for females in particular. The trend in mean CD4 counts has shown positive growth. Females no longer shoulder a disproportionate burden of disease.

OBJECTIVE: To estimate the global cost of establishing and operating the educational and refractive care facilities required to provide care to all individuals who currently have vision impairment resulting from uncorrected refractive error (URE). METHODS: The global cost of correcting URE was estimated using data on the population, the prevalence of URE and the number of existing refractive care practitioners in individual countries, the cost of establishing and operating educational programmes for practitioners and the cost of establishing and operating refractive care facilities. The assumptions made ensured that costs were not underestimated and an upper limit to the costs was derived using the most expensive extreme for each assumption. FINDINGS: There were an estimated 158 million cases of distance vision impairment and 544 million cases of near vision impairment caused by URE worldwide in 2007. Approximately 47 000 additional full-time functional clinical refractionists and 18 000 ophthalmic dispensers would be required to provide refractive care services for these individuals. The global cost of educating the additional personnel and of establishing, maintaining and operating the refractive care facilities needed was estimated to be around 20 000 million United States dollars (US$) and the upper-limit cost was US$ 28 000 million. The estimated loss in global gross domestic product due to distance vision impairment caused by URE was US$ 202 000 million annually. CONCLUSION: The cost of establishing and operating the educational and refractive care facilities required to deal with vision impairment resulting from URE was a small proportion of the global loss in productivity associated with that vision impairment.

OBJETIVO: Estimar el coste global del establecimiento y funcionamiento de las instalaciones educativas y de atención médica refractiva necesarias para proporcionar atención médica a todas las personas que padecen en la actualidad una discapacidad visual causada por un error de refracción no corregido (ERNC). MÉTODOS: El coste global de corregir los ERNC se calculó utilizando datos sobre la población, la prevalencia de ERNC y el número de especialistas en atención médica refractiva existentes en cada uno de los países, el coste del establecimiento y funcionamiento de programas educativos para especialistas y el coste del establecimiento y funcionamiento de las instalaciones de atención médica refractiva. Los supuestos aceptados garantizaron que no se subestimaran los costes y el límite máximo para los costes se obtuvo utilizando el extremo más caro para cada supuesto. RESULTADOS: Se calcularon 158 millones de casos de discapacidad en la visión de lejos y 544 millones de casos de discapacidad en la visión cercana causados por ERNC en 2007. Se necesitarían aproximadamente 47 000 refraccionistas clínicos funcionales adicionales a tiempo completo y 18 000 ópticos para proporcionar servicios de atención médica refractiva a esas personas. El coste global de formar al personal adicional y de establecer, mantener y administrar las instalaciones de atención médica refractiva necesarias se estimó en unos 20 000 millones de dólares estadounidenses (US$) y el coste límite superior se estimó en 28 000 millones de dólares estadounidenses. La pérdida estimada en el producto nacional bruto global debido a la discapacidad en la visión de lejos causada por ERNC fue de 202 000 millones de dólares estadounidenses anuales. CONCLUSIÓN: El coste del establecimiento y funcionamiento de las instalaciones de atención médica refractiva necesarias para tratar la discapacidad visual resultante de ERNC fue una pequeña proporción de la pérdida global en productividad asociada con esa discapacidad visual.

OBJECTIF: Estimation du coût global de mise en place et de fonctionnement des établissements de traitement de la réfraction et de formation nécessaires pour fournir des soins à toutes les personnes souffrant actuellement de troubles de la vision résultant d'une erreur de réfraction non corrigée. MÉTHODES: Le coût global de la correction des erreurs de réfraction a été estimé à l'aide des données sur la population, de la prévalence de la pathologie et du nombre de professionnels de la réfraction existant dans les différents pays, des coûts de mise en place et de fonctionnement des programmes éducatifs pour les praticiens et du coût de création et de gestion des établissements de soins pour la réfraction. Les hypothèses retenues veillaient à ce que les coûts ne soient pas sous-estimés et un seuil de coûts maximum a été calculé en utilisant les cas de figure les plus chers pour chaque hypothèse. RÉSULTATS: On a estimé à environ 158 millions le nombre de cas de déficience de la vision de loin et à 544 millions le nombre de cas de déficience de la vision de près due à un trouble de la réfraction dans le monde entier en 2007. Environ 47 000 nouveaux réfractionnistes cliniques en exercice à temps plein et 18 000 centres ophtalmiques seraient nécessaires pour fournir des services de soins de la réfraction à ces patients. Le coût global de formation du personnel supplémentaire et de mise en place, d'entretien et d'exploitation des installations de soins de réfraction nécessaires a été estimé à environ 20 milliards de dollars américains (US $) et le coût maximum était de 28 milliards US $. La perte estimée de produit intérieur brut mondial en raison de troubles de la vision de loin causés par la réfraction était de 202 milliards US $ par an. CONCLUSION: Le coût de mise en place et de fonctionnement des établissements de soins de la réfraction et de formation nécessaires pour faire face aux troubles de la vision résultant de pathologies de la réfraction représente une faible proportion de la perte globale de la productivité associée à cette déficience visuelle.

OBJECTIVE: To estimate the potential global economic productivity loss associated with the existing burden of visual impairment from uncorrected refractive error (URE). METHODS: Conservative assumptions and national population, epidemiological and economic data were used to estimate the purchasing power parity-adjusted gross domestic product (PPP-adjusted GDP) loss for all individuals with impaired vision and blindness, and for individuals with normal sight who provide them with informal care. FINDINGS: An estimated 158.1 million cases of visual impairment resulted from uncorrected or undercorrected refractive error in 2007; of these, 8.7 million were blind. We estimated the global economic productivity loss in international dollars (I$) associated with this burden at I$ 427.7 billion before, and I$ 268.8 billion after, adjustment for country-specific labour force participation and employment rates. With the same adjustment, but assuming no economic productivity for individuals aged > 50 years, we estimated the potential productivity loss at I$ 121.4 billion. CONCLUSION: Even under the most conservative assumptions, the total estimated productivity loss, in $I, associated with visual impairment from URE is approximately a thousand times greater than the global number of cases. The cost of scaling up existing refractive services to meet this burden is unknown, but if each affected individual were to be provided with appropriate eyeglasses for less than I$ 1000, a net economic gain may be attainable.

OBJETIVO: Estimar las pérdidas potenciales de productividad económica mundial asociadas a la actual carga de discapacidad visual por errores de refracción no corregidos (ERNC).? MÉTODOS: Se partió de supuestos prudentes y de datos demográficos, epidemiológicos y económicos de ámbito nacional para estimar las pérdidas de producto interno bruto ajustado por la paridad del poder adquisitivo (PIB ajustado por PPA) correspondientes a todas las personas con deterioro de la visión o ceguera, así como a las personas con visión normal encargadas de proporcionarles ayuda de manera informal. RESULTADOS: En 2007, unos 158,1 millones de casos de discapacidad visual se debían a errores de refracción no corregidos o insuficientemente corregidos; de ellos, 8,7 millones correspondían a personas ciegas. La pérdida de productividad económica mundial en dólares internacionales (I$) asociada a esa carga de morbilidad se ha estimado en I$ 427 700 millones y en I$ 268 800 millones antes y después, respectivamente, de ajustar en función de la tasa de actividad y la tasa de empleo de cada país. Con esos mismos ajustes, pero suponiendo una productividad económica nula para las personas > 50 años, estimamos que las pérdidas potenciales de productividad ascendían a I$ 121 400 millones. CONCLUSIÓN: Incluso empleando los supuestos más prudentes, la pérdida total estimada de productividad asociada a los ERNC es, en dólares internacionales, unas mil veces mayor que el número mundial de casos. Se desconoce el costo que supondría la ampliación de los actuales servicios de errores de refracción para poder afrontar esa carga, pero si cada persona afectada recibiera gafas apropiadas por menos de I$ 1000 podría obtenerse un beneficio económico neto.

OBJECTIF: Estimer la perte potentielle de productivité économique dans le monde associée à la charge existante de déficiences visuelles dues à des défauts de réfraction non corrigés. MÉTHODES: Des hypothèses conservatrices et des données démographiques, épidémiologiques et économiques nationales ont été utilisées pour estimer la perte de produit intérieur brut ajusté en parité du pouvoir d'achat (PIB ajusté en PPA) pour l'ensemble des individus affectés d'une déficience visuelle ou aveugles et pour ceux jouissant d'une vue normale, qui leur dispensent des soins de manière informelle. RÉSULTATS: En 2007, on estimait à 158,1 millions le nombre de cas de déficiences visuelles résultant de défauts de réfraction non-corrigés ou sous-corrigés, parmi lesquels 8,7 millions d'aveugles. Nous avons évalué la perte de productivité économique mondiale en dollars internationaux (I $) associée à cette charge à I $ 427,7 milliards avant ajustement pour la participation à la force de travail du pays et pour les taux d'emploi et à I $ 268,8 milliards après cet ajustement. En pratiquant le même ajustement, mais en prenant pour hypothèse une productivité économique nulle pour les personnes de 50 ans et plus, nous avons estimé la perte de productivité potentielle à I $ 121,4 milliard. CONCLUSION: Même en se fondant sur les hypothèses les plus conservatrices, on estime que la perte de productivité, en dollars internationaux, associée aux déficiences visuelles résultant de défauts de réfraction non corrigés est approximativement mille fois plus élevée que le nombre mondial de cas de tels défauts. Le coût du passage à l'échelle supérieure des services de réfraction existants pour faire face à cette charge n'est pas connu, mais la délivrance d'une paire de lunettes appropriée pour moins de 1000 I $ à chaque personne affectée devrait se traduire par un gain économique net.
